This year, Wells Fargo announced that due to a error in worker judgement 5300 employees were terminated from their employment.  It is clear that there was some rather large problem and that the solution to a large problem is to fire a large amount of "bad" employees.   
I just have one question:  What did they fix?

What could possibly be wrong with asking worker to stop when unsure?  It seems - I said seems - like such a good and right thing to say to workers...
but is it?
I wonder if we should change this advice to start when sure?  Isn't it easier to know when you are ready to do work and opposed to being psychic enough to know when some surprise event will lead to failure.
