On Novemeber 22, 2016 a school bus crashed in Chattanooga, Tennessee.  Six children were tragically taken from thier families, their communities, and the world.  Was it a bad driver?  Or is it a bad system - a system that is built around two very fragile operator dependancies - perfect school bus drivers and perfect motorist surrounding the bus?
This safety moment was painful to record - and I will bet it will be painful to hear - but listen and think about what this accident should mean for all of us.

We have for too long allowed a Western Economic definition of risk to rule our businesses, our thinking, and our system reliability programs.  
What would you say if I told your the current definition we use to describe risk is probably over simplistic and not very accurate.  Risk is not probability times severity.  Sorry safety guys - you have been sold some snake oil.   
Risk must be better defined to include variability and complexity.  Risk is not stable.  Risk is not permanant.  Risk is not linear.   Sorry.
Risk is more about defining the amount of uncertainty the worker will face.

The biggest problem we have in making our system reliable is the bias we have that we know the answer...
When we think we know the answer we don't look at the problem.   The safety Pyramid is a great example of safety mythology becoming hardwired in to our safety thinking.  It is not right.  It is not empirical.  It is just made up - and yet I'll bet in your company there are folks right now who think that if they stop a first aid they will have stopped a fatality.  
Ahhhhhhh  Dogma.... the sweet belief that we have it right.  It totally gets in the way of thinking, learning, and changing.
