We don't reduce human error.  We really can't.
We don't write perfect procedures.  We don't know how.
We don't have perfect equipment.  That is not possible.

How much do you really know about probability?  Would you be angry if I told you that your stoplight chart to determine probability is wrong.  Events either happen or don't happen...there is no in-between...none, zero, nada, zip, zilch, nil, zero.   
All the time we spend guessing how likely it is an event will happen is not very valuable to our workers or our organization.  We know, and have known for a long time, that workers don't really accurately guess probability.   I think I am safe.  I am not sure you are safe.  Both ideas are wrong.

This safety moment is by request...no actually by about a million requests.  This is a whole safety moment on my personal favorite safeguard, barrier, defense, you name it.  This safety moment is about rumble strips.    
Rumble strips are an excellent way to talk about how must create and manage systems that don't deny human error.   We don't drive error out of our organizations.  We build error in to our systems and processes.  We build systems that are able to handle variability from human operators - because we constantly have variability from human operators.

I was sitting around one afternoon talking to a group of friends.  We were chatting about resilience and reliability, safety and DevOps...you can maybe guess where we were.   David Woods told a story about always having "one more orbit around the earth" and how NASA looked at the idea of recoverability.
I was listening carefully because I struggle with what to call what we do....do we help add defenses and controls, safeguards and barriers, or do we give the organization capacity to fail safely?  In a way, a very real way, what we do is try to give workers "one more orbit around the work."  
That's when the great Dr. Woods said these words, "Graceful Exstensibility!"  That is what we want....now how do we do it?

OK.  Fasten your seat belts...I am about to say something that I never imagined I would ever say.  We are putting too much emphasis on prevention.  So much emphasis that we are not managing safeguards and controls very well.  We have talked ourselves in to believing that every accident can be prevented.  We have sold this idea to people so well that they actually believe it.   
The problem is when a bad thing happens - our workers are caught with their symbolic pants down.   Think of the automotive industry, think of all the defenses that are in a new car, and ask yourself this questions, "Should we be asking drivers not to crash, or managing the outcomes of the crashes we know will happen?"
