Safety Podcast, New Safety, Safety Learning, Human Performance, Safety II, Operational Learning
I was sitting around one afternoon talking to a group of friends. We were chatting about resilience and reliability, safety and DevOps...you can maybe guess where we were. David Woods told a story about always having "one more orbit around the earth" and how NASA looked at the idea of recoverability.
I was listening carefully because I struggle with what to call what we do....do we help add defenses and controls, safeguards and barriers, or do we give the organization capacity to fail safely? In a way, a very real way, what we do is try to give workers "one more orbit around the work."
That's when the great Dr. Woods said these words, "Graceful Exstensibility!" That is what we want....now how do we do it?
Thanks for listen to the podcast. You are making this baby go!

OK. Fasten your seat belts...I am about to say something that I never imagined I would ever say. We are putting too much emphasis on prevention. So much emphasis that we are not managing safeguards and controls very well. We have talked ourselves in to believing that every accident can be prevented. We have sold this idea to people so well that they actually believe it.
The problem is when a bad thing happens - our workers are caught with their symbolic pants down. Think of the automotive industry, think of all the defenses that are in a new car, and ask yourself this questions, "Should we be asking drivers not to crash, or managing the outcomes of the crashes we know will happen?"

There are two different times for workers. There is time to think. There is time to act. You don't really do these two functions at the same time. When we don't give time to do one or the other of these times...reliability suffers and suffers greatly.
This episode is very interesting and very important. Listen carefully and see what you think.
Thanks for listening to the podcast. You are a huge part of this little experiment's success.

This week's PreAccident Podcast episode is all about the place where manufacturing and reliability meet...and it is so much bigger than quality, safety, production, and operations.
Catherine Warner is a Senior Safety Leader in manufacturing and brings to the podcast microphone a depth and understanding of what it is like to shift thinking towards managing success through reliability. You will get much value from this podcast.

Bob is a long time safety thinker and leader. His approach to safety is a bit different then most people. Bob discusses how he thinks about his role and shares how you might think about your role as a safety person.
This is a great safety moment. Think how about how you view yourself. Success is only ours when other succeed. That seems like an important thought.
Thanks Bob...and thank you for being a part of the PreAccident Podcast. It only works because you listen.

This episode is a gift from Oil and Gas Denmark. Peter Burkill was the pilot of the ill-fated BA 038. He has a story to tell and does he ever tell it. Listen carefully, there is much here.
British Airways Flight 38 (call sign Speedbird 38) is a scheduled flight operated by British Airways from Beijing, China to London, United Kingdom. On 17 January 2008, local time 12:32 GMT, the Boeing 777 used for the flight, having completed the 8,100-kilometre (4,400 nmi; 5,000 mi) trip, crash landed just short of the runway at its destination.[1][2][3] There were no fatalities but from the 152 people on board, 47 people sustained injuries, one of them serious.[4] The 150-tonne aircraft was the first Boeing 777-200ER to be written off in the model's history,[5][6] and the first hull loss of any Boeing 777. Safety Podcast, Safety Program, Investigations, Human Performance, Safety Differently, Operational Excellence
This is the most important safety moment in a while.
As goofy as this sounds - we tend to think of an investigation as a "fix." Investigations are never fixes...investigations are done for one reason: To learn and improve (that seems like two reasons - just go with it).
Corrective actions fix. Corrective actions are the result of learning. Stop saying your company investigates in order to stop re-occurance... that is wrong.
Thanks for listening.

Today's episode is a discussion between Legal and Safety...Something that I try to normally avoid - because it never goes quite the way I want it to go. But today is different, this conversation goes really, really well. What a surprise!
We have to learn to invite Legal to the table earlier in our movement an process towards the new view. It is time to know that if we talk to Legal about what we are doing in prevention - they are much better prepared to have conversations when we have to manage a failure. Makes Sense...

This safety moment just reminds us that expertise is everywhere in your organization. You are not the sole owner of good ideas. You don't really know how work is done. You don't have a lock on operational knowledge.
The workers have an expertise that you can never have. User of your systems know things about your systems you simply don't know.
Knowing this makes life better. Listen to this episode and enjoy! Thanks for making the podcast great!
