Silveria, Administrator of the United States Air Force Acadamy gave the entire organization a chance to improve - he could have punished, he could have sough revenge, he could have responded entirely emotionally - but he chose not to...he chose to respond by improvement.

You can have the best ideas in the world, but if you don't have a way to execute on those ideas they really don't matter much.
Much of our improvement failures happen because we have not taken the time to talk about and create a system to use to improve.  Seriously, improvement doesn't happen by magic or accident.  Build a way for improvement to happen - and it will happen.
